Minister’s Medal – Excellence in Health Quality & Safety Local Health Integration Network (LHIN) was recognized in November for helping seniors access the care they need at home so they can live more independently.  Nucleus’ SDL Program was part of the  team awarded the Minister’s Medal Honouring Excellence in Health Quality and Safety for providing community resources to seniors discharged from hospital so they can recover at home rather than a long-term care facility. 

The Supports for Daily Living program offers personal support, homemaking and attendant services in designated neighbourhoods and buildings. These services are available around the clock every day of the year. The program has been so successful that it is being replicated in many communities across the province. 

The Board of Directors recognizes all Nucleus staff who have made the SDL Program a major contributor in improving services to seniors in the Healthcare system.   There were over 140 applications and the SDL Program won the inaugural Minister’s Team Medal.  It was an honour to receive the first award which will reside at the MH LHIN’s office.
