Supports for Daily Living program provides
in home services for seniors.
Our Supports for Daily Living program helps frail, high-needs seniors with assistance with activities of daily living, e.g. bathing, light meal prep, dressing, in their own home. The program is funded by the Ministry of Health and Long-Term Care and there is no cost to eligible clients.
Our services are provided by skilled Care Coordinators and Personal Support Workers (PSWs).
Access to 24/7 personal support and assistance with essential activities of daily living, as well as safety checks. See below for a list of available services.
High needs seniors (65 and older), who live in Mississauga, Oakville and South Etobicoke.
Central Registry - our centralized intake program - helps connect you with the right care, at the right time, to meet your needs.

✓ Changing non-sterile dressings
✓ Dressing/undressing
✓ Help with eating

✓ Preparing meals/cooking/clean-up
✓ Changing bedding
✓ Light house cleaning
✓ Laundry
✓ Garbage disposal
We welcome self-referrals as well as referrals from hospitals, community partners, health care professionals, individuals and/or family members.

Service providers are approved by the Mississauga Halton Local Health Integration Network carry the Supports for Daily Living Approved Service Provider seal of approval.
