This year, Nucleus celebrated an important milestone - 40 years of service. While much has changed over the course of four decades, our relentless commitment to improving the lives of the people we care for, is unwavering. Throughout 2023-2024, we reflected on our past, celebrated our accomplishments and imagined what we could become in our future. Ultimately, how did we do in our strategic quest for securing the core, learn and explore and be ready for more?


Since 1983, Nucleus has had exponential growth in programming, geographic-reach and staffing complement. From the 14 founding clients to now, thousands annually, Nucleus stands as a leader in modernized, integrated, compassionate home-based care in our region. Further, we create a lifeline for clients and families navigating the health system by connecting them to the best supports to meet their whole-person needs through our Centralized Intake program. For the last 10 years, it has also been a leader in training and educating personal care staff, regulated professionals and family caregivers in the community sector through the Regional Learning Centre (RLC). The programming is elevating the standards of care across all service provision, allowing people to live and thrive in their communities, as long as they wish. As you’ll read in this report, there are many more highlights along the way.


2023-2024 will be remembered as the year we improved our clients’ and staff’s experience. In August, through the Family Portal, we connected clients and caregivers to their own digital information. The portal provides access to timely and tangible information to improve their quality of life, based on their own needs. Further, we transitioned to new digital assets creating a powerful platform from which to launch Snowflake, PowerBI, ADP and Beanworks; collectively enabling us to expand access to information and empower our stakeholders. From a regional perspective, the continued expansion of Adult Day Services (ADS), in partnership with CareDove, has enabled us to assess and connect 681 clients to their preferred ADS program and reduced the waitlist by nearly 70% (from over 1400 to 450 people). By consolidating wait-list information across 22 providers, we minimized single caregivers and care recipients from experiencing multiple transition points across the system to just one.


This year also saw our continued commitment and perseverance of equity, diversity and inclusion. Through leading an experience-based education series for nearly 600 providers across Central Region, embedding culturally-sensitive meals through the food programs and cultivating staff-driven improvements, we remain in pursuit of equity across all facets of our organization and communities we serve. In May 2023, three years into our leadership journey, we were humbled to receive recognition from the Canadian College of Health Leaders with the Healthcare Leadership Award for outstanding contribution to an organization, community and health system.


Aspiring for excellence, every time, remains our north star, and the truth remains that our transformation journey has only begun. Our new vision of reimagining community and care, together, is the culmination of our experience to date that laid the foundation for future growth, performance and innovation beyond traditional community care thinking. We’re solutioning for what a person-centred health care system can be through our new mission to “enable adults to live with independence and a high quality of life in their communities, by delivering extraordinary care at home.”


When we started, the ‘art of possible’ made us envision endless opportunities for Nucleus. Thanks to the strong commitment from our talented and committed Board of Directors, dedicated staff, and our many stakeholders, including Ministry of Health, Ontario Health, Ontario Health Teams, community partners and collaborators, our commitment towards reimaging care in the community is catapulting us towards exceeding excellence, every time.

Effective November 30, 2021 all Nucleus staff, volunteers and contractors (including agency staff) must be fully vaccinated to be scheduled for work and deliver care. This is to ensure both client, staff and community safety against COVID-19.

Dr. David Burt is an immunologist with more than 30 years research experience in universities and vaccine companies. He has led scientific teams involved in the development of vaccines against various infectious diseases including influenza and SARS. He received his Ph.D. in Immunology from the University of Birmingham, UK and a BSc. in Biological chemistry from Essex University, UK. David received the African Canadian Achievement Award in Science in 1997 and the Harry Jerome Award for Health Sciences in 2006 for his professional work and promotion of science in the community. Currently, Dr. Burt is President of D. S. Burt, R&D Consulting, offering independent consulting advice in biotechnology and vaccine research and development. Dr. Burt is a member of the City of Toronto’s Black Scientist Task Force on Vaccine Equity whose mandate is to engage the communities most impacted by the pandemic around issues related to COVID-19 and vaccines.
